Implications of Genetic Testing in Dilated Cardiomyopathy
Background – Genetic analysis is a first-tier test in dilated cardiomyopathy (DCM). Electrical phenotypes are common in genetic DCM but their exact contribution to the clinical course and outcome is unknown. We determined the prevalence of pathogenic gene variants in a large unselected DCM population, and determined the role of electrical phenotypes in association with outcome.
Methods – This study included 689 DCM patients from the Maastricht Cardiomyopathy Registry, undergoing genetic evaluation using a 48 cardiomyopathy-associated gene-panel, echocardiography, endomyocardial biopsies and Holter monitoring. Upon detection of a pathogenic variant in a DCM patient, familial segregation was performed. Outcome was defined as cardiovascular death, heart transplantation, heart failure hospitalization and/or occurrence of life-threatening arrthymias.
Results – A (likely) pathogenic gene variant was found in 19% of patients, varying from 36% in familial to 13% in non-familial DCM. Family segregation analysis showed familial disease in 46% of DCM patients who were initially deemed non-familial by history. Overall, 18% of patients with a non-genetic risk factor had a pathogenic gene variant.
Almost all pathogenic gene variants occurred in just 12 genes previously shown to have robust disease association with DCM. Genetic DCM was independently associated with electrical phenotypes such as atrial fibrillation (AF), non-sustained ventricular tachycardia (NSVT) and AV-block (AVB), and inversely correlated with the presence of a left bundle branch block(p<0.01). After a median follow-up of 4 years, event-free survival was reduced in genetic versus non-genetic DCM patients(p=0.01). This effect on outcome was mediated by the associated electrical phenotypes of genetic DCM(p<0.001).
Conclusions – One in five patients with an established non-genetic risk factor or a non-familial disease still carries a pathogenic gene variant. Genetic DCM is characterized by a profile of electrical phenotypes (AF, NSVT and AVB), which carries increased risk for adverse outcomes. Based on these findings, we envisage a broader role for genetic testing in DCM.

Genetictesting for inherited retinal degenerations: Triumphs and tribulations
- Inherited retinal degenerations (IRDs) are a genotypically and phenotypically diverse group of conditions. Great strides have been made toward identifying the genetic basis for these conditions over the last 30 years-more than 270 different genes involved in syndromic and nonsyndromic forms of retinal dystrophies have now been identified.
- The identification of these genes and the improvement of clinical laboratory techniques have led to the identification of the genetic basis of disease in 56-76% of patients with IRDs through next generation sequencing and copy number variant analysis. Genetic testing is an essential part of clinical care for patients affected with IRDs and is required to confirm the diagnosis, understand the inheritance of the condition, and determine eligibility for gene-specific treatments or clinical trials.
- Despite the success achieved in determining the genetic cause of these conditions, several challenges remain, which must be considered when providing genetic testing and genetic counseling to patients. For this reason, an integrated team of ophthalmic and genetic clinicians who are familiar with these challenges is necessary to provide optimal comprehensive care to these patients.
A multicenter, prospective, blinded, nonselection study evaluating the predictive value of an aneuploid diagnosis using a targeted next-generation sequencing-based preimplantation genetictesting for aneuploidy assay and impact of biopsy
Objective: To determine the predictive value of an aneuploid diagnosis with a targeted next-generation sequencing-based preimplantation genetic testing for aneuploidy (PGT-A) assay in prognosticating the failure of a successful delivery.
Design: Prospective, blinded, multicenter, nonselection study. All usable blastocysts were biopsied, and the single best morphologic blastocyst was transferred before genetic analysis. Preimplantation genetic testing for aneuploidy was performed after clinical outcome was determined. Clinical outcomes were compared to PGT-A results to calculate the predictive value of a PGT-A aneuploid diagnosis.
Setting: Fertility centers.
Patient(s): Couples undergoing their first in vitro fertilization cycle without recurrent pregnancy loss, antral follicle count < 8, or body mass index ≥ 35 kg/m2.
Intervention(s): None.
Main outcome measure(s): The primary outcome was the ability of the analytical result of aneuploid to predict failure to deliver (clinical result). A secondary outcome was the impact of the trophectoderm biopsy on sustained implantation.
Result(s): Four hundred two patients underwent 484 single, frozen, blastocyst transfers. The PGT-A aneuploid diagnosis clinical error rate was 0%. There was no difference in sustained implantation between the study group and an age-matched control group, where biopsy was not performed (47.9% vs. 45.8).
Conclusion(s): The PGT-A assay evaluated was highly prognostic of failure to deliver when an aneuploid result was obtained. Additionally, the trophectoderm biopsy had no detectable adverse impact on sustained implantation.

One study shows high expression of Podoplanin mRNA in placenta, lung, skeletal muscle, and heart, and weaker levels in brain, kidney, and liver. Podoplanin is the ligand for Ctype lectin like receptor 2 (CLEC2). Their association is dependent on sialic acid on Oglycans of Podoplanin. Through its association with CLEC2, Podoplanin induces platelet aggregation and tumor metastasis. Podoplanin is also necessary for lymphatic vessel formation, normal lung cell proliferation and alveolus formation at birth. |
